Since I prepare the MIDI files I use myself, I almost always put the count-in in the file itself. It would be a little bit of work but you should be able to do that with MIDI files you get elsewhere. I only looked at the file quickly, so I may have missed something. To me it looks like there are 3 MIDI channels. 1 is left hand, 2 is right hand, and 3 is a section of the song with both hands in the same channel. I think you could set up all the channels to be played and use Synthesia's hand split...

For the descending notes try 1-2-1-2-5 where the thumb crosses under to play the Eb. You'll probably have to bend your wrist back and maybe play with the side of your thumb. My concern is that trying to play both of the last two notes with 5 could be choppy.

I think this is going to depend on your hand size. I don't have a keyboard available to try this on but I'd try 4-3-2-1-2-3-4-5 for starters. AFAIK MuseScore does export MIDI files that include the key signature information, although not the major/minor information as I recall. Have you tried simply importing the MIDI file into MuseScore and then exporting a new MIDI file?

Yes, all this information is extracted from the MIDI file itself. You don't have a bogus MIDI, you have a MIDI that isn't well structured for reverse engineering the notation.

I am afraid the answer of hire a piano teacher, take a piano class, or find a self study book or website is about the best you are going to get. Synthesia is NOT a piano tutorial. It is a practice tool that can help with whatever form of piano study you are doing.
