Which version of macOS is your computer running? I remember there was an old bug in the built-in Quicktime MIDI synth that made the MIDI nearly silent. (You could turn the system volume all the way up to just barely be able to hear anything.)
And what do you have enabled under "Music Output" on the Settings screen inside Synthesia?
