- Your own, original work submitted under some license that permits free private use (I recommend public domain or Creative Commons).
- Adapted from works created prior to 1923. This goes for both the composition and arrangement. This means you cannot post MIDI versions of recent songs.
Your best bet would be to find someone to transcribe the notes for you, not a hard task for an experienced piano player with decent proficiency handling sheet music.

Depends on the conditions under which you obtained the PDF. Typically sold sheet music does not allow for further reproduction or distribution. But what you do with it for yourself will probably not interest anyone.
You could always say that you played it from the sheet and recorded the midi yourself.

It may be legal, but sounds unlikely as the offering party would have to have permission by the artist / recording company / rights holder.
Also "legal" is a very vague term, as you may live in a totally different jurisdisction then the rights owner / download site operatorer etc.

This cannot be done. You can turn a MIDI File into Mp3 very easy but an MP3 to MIDI. I've never seen it done successfully except perhaps for single note MP3.
